Mission Support Program
Medical Authorization and Control
Medical authorization and control is provided by the TMI Medical Director, and/or his designees. This is usually accomplished via one or more of three (3) vehicles; Advanced Written Protocols, On-Line Medical Direction, and On-Scene Medical Control.
Tactical Medics International currently utilizes several written protocols (sometimes referred to as Standard Operating Procedures [SOPs]). The current TMI protocols are an accumulation/composite of several existing protocols, including: U.S. Special Operations Command Tactical Medical Protocols for Special Operations Advanced Tactical Practitioners; the 75th Ranger Regiment Trauma Management Team U.S. Army Ranger Medical Protocols; the Committee on Tactical Combat Casualty Care, Tactical Combat Casualty Care Prehospital Care in the Tactical Environment Protocols; TMI’s Standard (non-tactical) Prehospital Emergency Medicine Protocols; the Federal Bureau of Investigation Emergency Medical Support Program Prehospital Protocols; the Protective Medicine Branch, Special Operations Division, Federal Protective Service, Immigrations and Customs Enforcement, Department of Homeland Security, Federal Law Enforcement Medics Prehospital Treatment Protocols; and TMI’s own protocols for chemical restraint, and medical clearance for incarceration.
Each TMI Medic will be familiar with each of these protocols and receive training and authorization in each procedure contained in each protocol. TMI encourages their Medics to utilize any authorized procedure (i.e., mix and match protocols) to accomplish the medical mission and provide the most appropriate care for the patient. Tactical Medics International understands the need for their Medics to operate at the level of their training – with appropriate medical control and supervision.
On-Line Medical Direction is provided via the Mission Support Center (described below). TMI currently utilizes ten (10) qualified and licensed physicians, identified as “1st Tier Physicians” who operate under the guidance of the Medical Director of the Company. These physicians all have extensive training in primary care, emergency medicine, tactical medicine and military medicine. Many are former military physicians, some are current U.S. Military reservists, and most have experience as civilian tactical physicians who provide medical direction to medics supporting law enforcement agencies. Each physician is on a rotation and serves as the “on-call” physician for the Mission Support Center.
In addition to these 1st Tier Physicians, TMI has an extensive list of additional personnel including; Physicians, Physician’s Assistants, Advanced Registered Nurse Practitioners, and Nurse Anesthetists who can provide on-line medical direction and/or medical advice. These personnel have tremendous experience in military operations and comprise a wide range of medical specialties, expertise and unique credentials including; dive medicine, aerospace medicine, hypobaric (altitude) disorders, orthopaedic and soft tissue injuries, burns, blast and crush injuries, etc. Each of these specialists is available to assist the TMI Medic, and can be accessed via the TMI Mission Support Center.
On-Scene Medical Control - “Deployable Human Assets”

Tactical Medics International operates a Mission Support Center (MSC) 24 hours a day, 7 days a week, 365 days a year. This center is available to all TMI Medics and other identified personnel. This hotline serves many purposes, but is used mostly for on-line medical control (see Medical Authorization and Control). The MSC is staffed my a Mission Support Specialist who fields telephone calls, e-mails, and other communications and directs the appropriate course of action. Via the MSC, the Mission Support Specialist provides the Medic with direct access to a physician who can provide information or advice, as well as on-line verbal or written orders, and advanced medical direction. The Mission Support Specialist, via the MSC, also serves as a resource for providing additional medical assets and medical intelligence gathering. Other forms of mission support provided by the MSC includes; medical pre-planning and medical threat assessments.
Deployable Human Assets
TMI can supply Agencies with trained and licensed medical personnel, including physicians, physician’s assistants (PAs), paramedics, qualified tactical/combat medics, etc. These personnel will provide medical support to the Agency which does not have their own personnel (or with the specialized training required), or will assist, support or augment an Agency’s Medics or other personnel in providing on-site medical services. These medical professionals all have operational experience, and represent a wide range of medical specialties. TMI will supply these medical personnel along with the clinical (medical) equipment and supplies necessary to provide on-site field emergency and/or operational medical services.
TMI currently utilizes ten (10) qualified and licensed physicians, identified as “1st Tier Physicians” who can provide on-scene medical control. These physicians all have extensive training in primary care, emergency medicine, tactical medicine and military medicine, and are readily deployable. Many are former military physicians, some are current U.S. Military reservists, and most have experience as civilian tactical medics or physicians. Each physician is on a “deployment rotation schedule” which is coordinated by the Mission Support Center. |
